China 摘要! 目 的: 研究胰岛素 样生长因 子 受体( IGF R) 在大肠 息肉 和大肠癌 中的表达及其 意义。 方 法: 收集内 镜下 黏膜切除技术( EMR) 或黏膜下层 剥离 术(ESD) 完整切 除并经病理确诊的 32 例患者 的 47 个大肠 息肉 , 包括炎性息 肉 、 管 状腺瘤、 绒毛状腺瘤; 另 选 12 例正常结 肠黏膜组织 及 20例结 肠腺癌黏膜组 织作为对 照。 用 免 疫组化 SP 法 检 测 IGF IGF R的表达程度及其与 大肠癌 的相 关性。 结果: 经 Mann Whitney U 检验炎性息R 的 表 达, 分 析肉 IGF R 与其正常黏膜中 IGF R 比较差 异无 统 计学 意 义, P= 0 251; 经 MannWhitney U 检 验管 状 腺瘤、 绒毛 状腺 瘤IGF R 与其正常黏膜 IGF R 间差 异有统计 学意义, P 均< 0 001, 故管 状腺瘤、 绒毛状腺瘤 IGF R 水平 比 其正 常黏 膜高。IGF R 在正常对 照组、 炎性息肉 组、 管状腺瘤组、 绒毛状腺瘤 组和 大肠 癌组 5 组 病例 间 经 Spearman∀ s 双变量等 级相 关性 分析, 差 异 有 统 计 学 意 义, r = 0 574, P <0 001。 3 组之间从年龄、 性别 及病 例数差异无 统计学意 义, P> 0 05。 结 论: 管状腺瘤、 绒毛状腺瘤 IGF R 水平 比其 正常 黏膜高 ; IGF R 在 正常 组 织、 炎性息 肉 、 管状腺瘤、 绒毛状腺瘤 和 大肠 癌中 的表达依次增 加。 IGF R 在 评价息肉 处 于黏 膜增生 腺瘤 癌变的发展中起了 重要的作用 。中华肿瘤防治杂志, 2010, 17(18 ) :1407 - 1410[ ABSTRACT] OBJECTIVE: To study the expression and clinicalsignificance of IGF R in colorectal polyps and colorectal carcinoma. METHODS: A total of 47 colorectal polyps including inflammatory polypus, tubular adenomas and villous adenomas, from 32patients were collected through EMR and ESD, and confirmed bypathology. Furthermore, 12 normal people∀ s colorectal mucosa and20 cases of colon adenocarcinoma mucosa were selected as controlgroups. Immunohistochemistry ( SABC) w as employed to examinethe expression of IGF R. T he relationships between the degree oftheir expression and t heir dependability and colorectal carcinomaw ere analyzed. RESULTS: Through Mann Whitney U test, no significant difference was explored betw een the IGF R in the inflammatory polyps and IGF R in normal mucosa ( P= 0 251) . Thedifference bet ween IGF R in the tubular adenomas and villous adenomas and in normal mucosa was examined by Mann Whitney Utest showing a statistical significant difference ( P < 0 001 ) , soIGF R levels in t he tubular adenomas and villous adenomas weremore than t he level in the normal mucosa. Correlation of IGF R among normal control, inflammatory polyp, tubular adenoma, villous adenoma and colorectal cancer had statistical significance by Spearman∀ sdouble varianceinterclass correlation (r= 0 574, P< 0 001) . There wereno statistical significance by subgroup analysis of age, sex, and the number of cases in each varible (P> 0 05) . CONCLUSIONS: IGFR levels inthe tubular adenomas and villous adenomas are more than the level in thenormal mucosa. The expression of IGF R is increased one by one in normal tissue, infiammatory polypus, tubular adenomas, villous adenomasand colorectal carcinoma.
