(Department of Orthopaedics,No 6th People’s Hospital,Ningbo,Zhejiang,China 315040) Abstract Objective: to observe the clinical effect of Application of the posterior occipitocervical fixation technique in occipitocervical instability. Methods: From October 2001 to February 2004,21 patients with occipito-cervical instability or atlanto-axial instability underwent occipital-cervical fusion using AXISinternal fixation system with autologous bone grafts which included 7 cases of old atlantoaxial fractures and /with dislocation,6 cases of combination fracture of Jeffson fractures and Hangman fractures with atlanto-axial instability,4 cases of Jeffson fractures with occipitocervical instability,4 cases with complex occipitalcervical deformity. There were 3 type fixation methods: ⑴occipitocervical fixation of C2pedicle screws fixation in 7 cases,⑵occipitocervical fixation of C2pedicle screws and C3 lateal mass screws in 8 cases,⑶occipitocervical fixation of C2 and C3 pedicle screws fixation in 6cases. Results 21 patients were observed clinically and radiographically at at an average of 19 months after surgery. 20 patients got solid fusion radiologically and one patient had no fusion.Of the 5cases patients with neurologic deficits, all of them had a significant improvement from their myelopathy.No patient had a worse result after surgery.There were no instances of plate breakage and lossening of the screws. Conclusion Occipitocervical reconstruction by the combination of C2 pedicle screws or C2-3 pedicle screws or C2 pedicle screws and C3 lateral mass screws and occipitocervical plate systems can provide sufficient correction of malalignment in the occipitocervical region and achieve high fusion rate. Key words Upper cervical vertebra; occipital-cervical fusion;Internal fixation 各种原因引起的寰枢椎不稳或脱位均需要复位和稳定， 复位后多数采用寰枢椎内固定和融合， 如后路的 Gallie 和 Brooks 手术及 Magerl 手术， 但是对于某些严重的 C1、 2 联合性骨折， 陈旧性寰枢椎脱位或肿瘤、 炎症、 畸形等伴有脊髓压迫症且难以复位的患者， 上述内固定方法往往无法使用 。 对于此类患者目 前国内外均采用枕颈融合术。 我们自 2001 年 10月~2004 年 2 月采用枕颈固定技术治疗上颈椎不稳 21 例取得较好的效果。
