The effects of 7.5% NaCI/6% dextran 70 on coaguIation and pIateIet aggregation in humans ［J］ .JTrauma， 1992， 32： 40 - 44.!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!（本文编辑： 贺羽）文章编号： 1009 - 4237 （2004） 04 - 0305 - 01· 病例报告 ·内踝粉碎性骨折髂骨移植重建 1 例Reconstruction of internal malleolus of comminuted fracture with iliumtransplantation in one case李英华， 官正华（解放军 324 医院骨科， 重庆400020）收稿日期： 2003 - 12 - 15； 修回日期： 2004 - 01 - 15摘要： 使用髂骨移植重建内踝粉碎性骨折 1 例， 重建成功， 踝关节外形及功能恢复良好。关键词：内踝骨折；髂骨移植中图分类号： R 683.42临床资料伤员男性， 19 岁， 驾驶员。车祸中被汽车仪表顶压致右踝肿痛、 功能障碍 1 小时入院。入院查体：右内踝部软组织挫伤重， 踝关节拒活动。X 片显示：右内踝粉碎骨折， 骨折块大小不等 （约 6 块） ， 在 0.5 ~ 1.2cm 左右， 且有明显移位。诊断：右内踝粉碎性骨折。入院后， 先予石膏固定， 对症治疗， 肿胀消退后手术。手术中使用止血带。术中发现右内踝粉碎成碎骨块， 内踝韧带断裂散乱， 已无法复位内固定修复。征得伤员同意， 取左侧髂嵴中段髂骨块重建。术中将髂骨块修成内踝大小形状， 以髂嵴端对骨折面， 髂骨内板对着踝关节面， 把髂骨外板剥除， 露出松质骨面， 以备韧带附丽。正确安放好髂骨块后， 用 2 枚可吸收加压螺钉穿过骨折面固定，然后将断裂的韧带理顺附于髂骨松质骨面上， 用 1 号丝线牵文献标识码： B引缝合， 分层缝合切口。术毕石膏托固定。照片显示：所造内踝逼真， 踝关节达到解剖复位。经抗感染治疗， 切口 14 天愈合拆线， 继续以管形石膏固定踝关节于功能位 2 个月。骨折临床愈合后， 开始踝关节功能锻炼， 并逐渐负重行走。1 年后复查踝关节外形正常， 活动范围接近对侧踝关节。短距离行走无碍， 长距离行走后有轻度酸胀痛， 已恢复驾驶员工作。讨内踝是踝关节的重要组成部分， 一旦骨折错位， 可产生踝关节不稳。一般内踝骨折手术治疗时可行切开复位加压螺钉或钢针钢丝内固定；对于碎裂骨折， 可行切开复位细钢针固定。本例内踝骨折， 由于暴力特殊， 骨折粉碎非常严重，无法整复内固定。采用髂骨移植重建内踝， 能恢复踝关节的完整性， 且髂骨可以依据内踝的形态造型， 髂骨的内侧面光滑， 与正常的关节面接近， 符合踝关节的生物力学原理。术后随访， 疗效满意。术中采用可吸收螺钉内固定， 省略了第二次手术的痛苦。但重建内踝时勿忘修复三角韧带， 否则也会影响手术的疗效。论（本文编辑： 贺羽）·503·创伤外科杂志 2004 年第 6 卷第 4 期J Trauma Surg， 2004， VoI.6， N0.4
