中国脊柱脊髓杂志 2014 年第 24 卷第 2 期 Chinese Journal of Spine and Spinal Cord，2014，Vo1．24 ，No．2 l 3 3 脊髓损伤 1 8 个月后 患者残疾心理 适应能力的问卷调查分析 徐 艳 文 ，Chow S．L am。，唐丹 ( 1 广东省工伤康复中心 职业康复研究中心 510440 广州市 ； 2 Institute of Psychology，Illinois Institute of T echnology ，Illinois，U SA ) 【摘要】目的：调查及分析脊髓损伤 18个月后患者残疾心理适应水平及其影响因素。方法：收集我院 2006 年 3 月～2008 年 12 月住院的脊髓损伤患者 78 例及来 自工伤康复互助组 织的脊髓损伤患 者 83 例 ，共 16 1 例 ，受伤 时年龄 18 岁 以上 ( 18～62 岁 ，平 均 36 岁) 且 伤后时 间超过 18 个月 ( 18～35 个 月 ，平 均 27 个月 ) 。问卷调查 在 2009 年 1 月一4 月完成 ，29 例 通过面谈 、132 例通过 电话访谈方式 完成 问卷 调查。问卷调查内容包括世界卫生 组织残疾评估量表 、残疾接受量 表 、Rosenberg 自我尊重 量表 、抑郁 量表 、自我效能量表及 Zimet 多维觉 察社会 支持量表。应用相关性分析及多元线性回归的方法分析影响残疾心理适应的因素。结果 ：脊髓损伤患者的残疾 心理适应与 自评疼痛程度及 WHO 自我残疾评估呈一 般负相关 (r 分别 为一0．28、一0．40 ，P<0．01) ，与 自我 效能及 社会支持呈一 般正相关 (r 分别 为 0．45、0．36 ．P<0．0 1) ，与 自我尊重呈 中度至高度 正相关 (r=0．65 ，P<0．01) ，与抑 郁呈中度至高度 负相关 (r=一0．66，P<0．01)。线性 回归分析结果 显示 抑郁 、自我尊重及疼痛程度是影响脊髓损伤 患者残疾适应 的三个主要 因素 (P <0．01) 。结论 ：脊髓损 伤 18 个月后 患者 残疾 适应态度受多 因素影 响 ，其 中抑 郁 、自我尊重及疼痛程度是最重要的影 响因素 【关键词 】脊髓损伤 ；残疾心理适应 ；康复心理 doi ：10．3969~．issn．1004—406X．2014．02．07 中图分类号：R683．2 文献标识码 ：A 文章编号 ：1004—406X ( 2014 )一02—0 133—06 A questionnaire survey on acceptance of disability of patients with spinal cord injury after 18 months of injury／X U Y anwen，C how S．L am，T ANG Dan／／Chinese J ournal of Spine and Spinal Cord，2014，24 (2)：133- 137，143 【A bstract】 Objectives： To survey and analyze the level of acceptance of disability and its risk factors for spinal cord injury(SCI) after 1 8 months of injury．M ethods：78 patients with SCI in our hospital from March 2006 to December 2008 and 83 cases with SCI fr om work injury mutual suppo~ group were reviewed retre— spectively，their ages at onset were above 18 years(18—62 years，average 36 years) and they suffered fr om in— j ury for more than 18 months(18- 35 months，average 27 months)．The survey ranged from J anuary to Apri l 20 09 ．Of them．29 were face—to—face i nte rv i ewed and 1 32 were tele phone i nterviewed．T he questi onnaire con— sisted of W orld Health Organization D isabilitv A ssessment Sc hedule 11 。 A cce ptance of Disability S cale R osenberg Sel f- E steem Scal e，C enter fo r E pi demi ologi c S tudi es Shoa D epression Scal e，G enerali zed S elf- E ffi — eacy Scale ，and Multidimensi onal S cale of Percei ved Social S uppo~ ．R esul ts ：T he acceptance of disabil ity had a fair degree of negative relationship with pain and W H O disability assessment f，：一0．28 and 一0．40， P<0．0 1， respectively)， and had a fair degree of positive relationship with self- efi cacy and social suppo~ (r=O．45 and 0．36．P<O．01．respectively)．It was also found that moderate to good positive relationship between acceptance of disability and self—esteem(r=0．65，P<0．01)，moderate to good negative relationship with depression(r=一0．66， P<0．01)． The linear regression result indicated that depression， self- esteem and pain intensity were the best predictors for acceptance of disability(P<0．01)．Conclusions：Multifactors may infl uence the acceptance of dis— abili ty fo r S CI．H owever，depression，self- percei ved pai n i ntensi ty and self- esteem are the best predictors fo r acceptance of di sabili ty ． 第一作者简 介：男 ( 1977一) ，硕士，主管康 复治疗师 ，研究方 向：工伤预防及职业康复 电话 ：(020)66600006—8866 E—mail：xudeng2@163．CON
