This studywas designed to investigate the roles of socioeconomicfactors and acculturation in teenage childbearing in thispopulation. Methods Logistic regression was used to ana-lyze the association of measures of acculturation (languagespoken at home, nativity, and age at immigration) andrespondents’ parents’ education with age at f i rst birth in astratif i ed sample of post-partum women in California.Results The unadjusted odds ratio for teenage birth forLatinas versus non-Latina Whites was 5.2 (95% CI 4.1–6.6). Nativity was not signif i cantly associated with teenbirth, but speaking Spanish at home was positively asso-ciated and immigrating at a later age was negativelyassociated with teen birth. Overall, these measures ofacculturation accounted for 17% (95% CI 8–28%) of thedifference in odds of teen birth between Latinas and non-Latina Whites. Higher levels of education among respon-dents’ parents had differentially protective effects acrossthe racial/ethnic groups. Controlling for disparities inrespondents’ parents’ education without changing its dif-ferential effects across racial/ethnic groups reduced theodds ratio for Latinas compared to non-Latina Whites by30% (95% CI 14–60%). Conclusion These f i ndings callinto question common assumptions about the protectiveeffect of acculturation on teen fertility and suggest thatimproving childhood socioeconomic factors among Latinasmay decrease teen childbearing.Keywords Latinas ? Teen pregnancyIntroductionBirths to adolescent mothers are associated with adverseoutcomes, including pregnancy complications, [1] lowereducational attainment, [2] and increased rates of sub-sequent poverty [3, 4] for the mother and increased risk ofneglect, [5] behavioral problems, [6] and low educationalachievement [7] for the child. Nationally, Latina adoles-cents have a higher birth rate than non-Latina Whites witha rate of 81 per 1,000, compared to 26 per 1,000 amongnon-Latina White adolescents [8]. Rates of adolescentchildbirth have been decreasing overall in the UnitedStates, but have decreased more slowly among Latinas.Between 1991 and 2005, teen birth rates among both non-Latina Whites and African Americans declined more than50% and 48%, respectively, yet the rate among Latinasdeclined by only 22% [8]. Decreasing the rate of teenpregnancy among Latinas, and eliminating the disparitiesbetween Latinas and non-Latina Whites, is recognized asa priority by the public health community, including theCenters for Disease Control and Prevention [9] and theNational Campaign to Prevent Teen Pregnancy [10].
